At Thames Valley Fertility we currently treat self-funded patients who have either self-referred themselves to us or been referred via their GP or fertility specialist, giving everyone the same expert care and attention.

NHS funding is granted by the Clinical Commissioning Group (CCG) and is subject to certain criteria dependent on the area you live in. We are currently in the process of setting up contracts with the local CCG’s to be able to start offering NHS funded treatment soon.

Medical referrals

Generally most patients will have already presented to their GP’s with their fertility concerns. Your GP is able to send us a referral to undertake any fertility investigations for you and as necessary start you on your fertility journey. You can simply request your GP to refer you on to us and they can then send us any supporting test results that you may have already had.

Alternatively you may have already been seen by a fertility specialist in a clinic or hospital, some of the local specialist clinics are led by our consultants and so they will easily be able to refer you through to us. As with GPs, as the patient you are able to request that your doctor makes a referral on your behalf to us.

Self-referrals

Alternatively, if you wish to privately fund your treatment you can self-refer to us by completing our Self-Referral Request (below).

If you have any test results already, please bring these along to your first consultation.

Referral letters to be addressed to:

Consultants Team Thames Valley Fertility
Clarrion House
Norreys Drive
Maidenhead
SL6 4BY

Or faxed to +44 (0) 1628 882418

If possible, your referral letter should include basic hormone profile blood tests, your medical and surgical history and a semen analysis result.

One of our Consultants will review your letter and offer you the most appropriate consultation.

NHS referrals

Eligibility for NHS funded treatment is based on, among other criteria; female age, BMI, whether both partners are non-smokers and the length of time a couple have been trying to conceive. Your GP should be able to advise you further on whether you are likely to meet the criteria for funding.

Although we do not at present have our own contracts set up to offer the complete NHS funded cycle at Thames Valley Fertility, we are able to offer to do all of your scans and cycle preparation with us in partnership with our sister clinic Oxford Fertility who have contracts to undertake NHS cycles. In this way of us acting as a satellite unit for Oxford we can keep as much of your treatment cycle as local as possible, with the requirement of going to Oxford for egg collection and embryo transfer only.

The referral for treatment must come either from your NHS GP or NHS Consultant.

After submission of the referral form below our Patient Services Team will follow up by sending you a registration pack in the post: (Fields marked with an asterix are compulsory, however for donors or patient's undertaking treatment independently please enter N/A in the partner sections).
